I. General Requirements
Adhering to a people-centered approach to health, with high-quality development as the central theme, and guided by the construction of the Qujing Regional Medical Center in Yunnan Province, we will strengthen the primary role and public welfare nature of public hospitals. We will uphold the integration of medical treatment and prevention, the combination of routine and emergency care, and the equal emphasis on traditional Chinese and Western medicine. Focusing on establishing and improving the modern hospital management system, we will enhance innovation in systems, technology, models, management, mechanisms, and governance, and accelerate the expansion and balanced regional distribution of high-quality medical resources.Strengthen the division of labor, collaboration, and operational integration between public hospitals at all levels and public health institutions. By 2025, a framework for high-quality development of public hospitals of all types and levels will be fundamentally established. The development approach of public hospitals will shift from scale expansion to quality improvement and efficiency enhancement; operational models will transition from extensive management to refined management; and resource allocation will shift from an emphasis on material factors to a greater focus on talent and technological factors, enabling the public to enjoy higher-quality and more efficient medical and health services.Accelerate the construction of infectious disease hospitals in Qujing City, Xuanwei City, Huize County, and Luoping County. General hospitals in other counties (districts) should complete the construction of relatively independent infectious disease wards to more effectively prevent and mitigate major epidemics and sudden public health risks, thereby providing strong support for accelerating the modernization of public health and promoting common prosperity.
II. Key Tasks
(1) Establishing a New System for High-Quality Development of Public Hospitals
1. Rationally allocate healthcare resources. Scientifically coordinate and plan the functional layout of public hospitals within municipal and county-level regions to promote the “restructuring and reorganization” of public hospitals across the city and the “enhancement and upgrading” of their service capabilities, thereby forming a development pattern characterized by distinct specialties, complementary expertise, differentiated development, and orderly competition.Fully leverage the technical, research, talent, disciplinary, and management strengths of the Qujing Center Hospital of Yunnan Provincial Regional Medical Center to establish a municipal-level cluster of key clinical disciplines. Guided by refined management, promote the balanced distribution and enhanced quality and efficiency of high-quality medical resources, and improve the service capacity and standards of existing public hospitals through disciplinary development and talent recruitment.Accelerate the joint construction of the National Regional Medical Center for Traditional Chinese Medicine by the Municipal Hospital of Traditional Chinese Medicine and the Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine, and continue to consolidate the achievements of Qujing as a National Advanced Demonstration City for Traditional Chinese Medicine. Strengthen the treatment capacity of the Yunnan Provincial Qujing Regional Medical Center and the Municipal Maternal and Child Health Hospital, effectively improving prevention and treatment capabilities in emergency medicine, oncology, cardiovascular and cerebrovascular diseases, orthopedics, hepatobiliary diseases, obstetrics and gynecology, pediatrics, and infectious diseases, thereby reducing the need for cross-regional medical treatment.(Responsible Units: Municipal Health Commission, Municipal Development and Reform Commission, Municipal Human Resources and Social Security Bureau, Municipal Finance Bureau; People’s Governments of all counties, cities, and districts, and the Qujing Economic and Technological Development Zone Management Committee shall assume responsibility in accordance with their respective duties. 2. Promote the upgrading and expansion of integrated medical consortiums. Vigorously advance the construction of integrated urban medical consortiums, improve their operational management mechanisms, and fully leverage the leading role of municipal-level hospitals within these consortiums. Explore the implementation of unified management of personnel, finances, and resources within integrated urban medical consortiums to promote their substantive operation. By 2025, establish three integrated urban medical consortiums.Relying on the Yunnan Provincial Regional Medical Qujing Center Hospital, actively explore integrated health service models for specialized conditions such as cardiovascular and cerebrovascular diseases, oncology, orthopedics, and rehabilitation. Provide residents with continuous “end-to-end” services covering disease prevention, treatment, convalescence, and health promotion, thereby enhancing municipal-level capabilities in the diagnosis and treatment of major diseases, the coordinated development of medical and preventive care, and the level of health management services.We will continue to deepen the cooperative models of medical consortiums between municipal and county-level public hospitals, promote the use of high-quality municipal medical resources to support the development of county-level hospitals, and strengthen guidance and support for the development of clinical disciplines and key specialties, innovation in clinical diagnostic and therapeutic technologies, and the implementation of research projects at county-level public hospitals. 3. Deepen the development of close-knit county-level medical consortiums. Streamline the establishment of 9 close-knit county-level medical consortiums, designating one public hospital in each county (city, district) as the lead hospital for the consortium. Strengthen the functions of the medical consortium management committees, continuously deepen the reform of per-capita bundled payment for medical insurance, and ensure the autonomy of medical consortiums in operations and management.Accelerate the construction of “five major resource-sharing centers”—covering medical testing, medical imaging, electrocardiography, pathology, and sterilization supply—that connect counties and townships, to promote the sharing and efficient utilization of high-quality resources within the county.Strengthen the development of leading disciplines, key specialties, and specialized disease centers at the lead hospitals; enhance the service capabilities of member units in emergency care, general practice, pediatrics, rehabilitation, nursing, and traditional Chinese medicine; and continuously advance the integration of resources, talent pooling, and service convergence within medical consortiums to elevate the level of medical services within the county.Based on service population, transportation conditions, and service capacity, guide township central health centers to develop into sub-centers for county-level medical services, achieving the service capacity and standards of a Grade II general hospital, and support township health centers (community health service centers) with mature conditions in establishing community hospitals.From 2022 to 2025, the proportion of patients seeking care at the primary level and the number of inpatient admissions within the county will steadily increase. By the end of 2025, the development model and institutional framework of the close-knit county-level medical consortium will be more mature and well-established,90% of township health centers across the city will meet the basic service capacity standards for township health centers, and the county-level treatment rate and the proportion of primary-level outpatient and emergency visits will remain stable at 90% and 65% or higher, respectively. 4. Enhance the level of prevention, control, and treatment of major infectious diseases. Strengthen the standardized development of public health departments in municipal hospitals, establish a mechanism for the coordinated development of medical and public health services, intensify reforms for medical-public health coordination between municipal public hospitals and the Municipal Center for Disease Control and Prevention, and promote the shift from a “disease-centered” approach to a “people’s health-centered” approach.Strengthen the infrastructure of the infectious disease treatment system, implement projects to enhance treatment capacity for major infectious diseases, and accelerate the construction of infectious disease hospitals in Qujing City, Xuanwei City, Huize County, and Luoping County. General hospitals in other counties (districts) shall complete the construction of relatively independent infectious disease wards to improve emergency medical response capabilities during major epidemics.Establish respiratory and critical care departments in all tertiary general hospitals across the city; comprehensively renovate fever clinics, isolation wards, and negative-pressure wards; and enhance the provision of equipment for oxygen therapy, monitoring, critical care, and resuscitation in isolation and observation areas. Establish standardized fever clinics and gastrointestinal clinics within secondary and higher-level medical institutions, ensuring that at least one general hospital in each county (district) has an infectious disease department.Promote the construction of a National Emergency Medical Rescue Base and a Traditional Chinese Medicine (TCM) Epidemic Prevention and Control Base, and establish one Qujing City Emergency Medical Rescue Team and one Qujing City TCM Epidemic Prevention and Control Team. Select several public spaces to serve as emergency treatment stations (sites) for public health incidents. (2) Fostering New Trends in the High-Quality Development of Public Hospitals
1. Strengthen the development of key clinical disciplines.Guided by the principle of “driving disciplinary development through specialty construction,” systematically enhance the development of key clinical specialties and disciplines. Implement plans for flagship, leading, distinctive, and promising disciplines, and advance the “Four Batches” discipline development initiative—namely, establishing a batch of flagship disciplines, developing a batch of key disciplines, cultivating a batch of distinctive disciplines, and supporting a batch of promising disciplines. Strengthen the development of talent pipelines, with a focus on diagnosis, treatment, research, and teaching in the fields of cardiovascular and cerebrovascular diseases, hepatobiliary diseases, orthopedics, oncology, and rehabilitation.By the end of 2025, we aim to establish 8 national-level key clinical specialties, 30 provincial-level key clinical specialties, and 50 municipal-level key clinical specialties. Within the Qujing Regional Medical Center of Yunnan Province, we will jointly build and share 10 specialized centers covering rehabilitation, clinical laboratory, medical imaging, electrocardiography, pathological diagnosis, molecular diagnosis, telemedicine, health management, research management, and emergency care.Key indicators reflecting healthcare service capacity—such as the CMI score, the number of Grade 3 and 4 surgeries, and their proportion—across all public hospitals at Level 2 and above in the city will continue to improve steadily. The CMI score for tertiary general hospitals will reach 1.15, with Grade 4 surgeries accounting for 16.4%; for maternal and child health specialty hospitals, the CMI score will reach 1.26, with Grade 4 surgeries accounting for 4.56%.The adoption rate of clinical pathways in tertiary hospitals has reached over 50%, and in secondary hospitals, it has reached over 70%, enabling the public to access advanced and satisfactory healthcare services nearby. 2. Enhance Scientific Research and Innovation Capabilities. Elevate high-level clinical medical standards and advance medical technology innovation. Encourage and support key clinical disciplines to engage in scientific research and innovation, and improve incentive and management systems—including research performance evaluations, rewards for technology transfer, and revenue distribution—to stimulate innovation vitality. Actively collaborate with leading domestic medical research institutions, engage in scientific research partnerships, and actively seek national and provincial-level science and technology projects and achievement awards.Promote the integration of industry, academia, and research, and support joint research efforts and the application of innovative technologies with universities, research institutes, and enterprises. Led by the Municipal Hospital of Traditional Chinese Medicine, establish a number of bases for clinical research and the incubation and transformation of research outcomes in traditional Chinese medicine. 3. Strengthen the Supportive Role of Information Technology. In accordance with Qujing’s “City Brain” and “Digital Qujing” strategic frameworks, we will intensify efforts to introduce and apply cutting-edge technologies, establish a comprehensive information management system, advance the high-standard development of regional medical informatization, and promote the coordinated and integrated development of health and medical big data across industries and regions.Adopting the “1 Platform, 3 Integrations, 3 Databases” model, we will build the “Healthy Qujing” big data platform to achieve mutual recognition of laboratory and imaging test results among hospitals at the secondary level and above. We will integrate the full-process regulatory information system for pharmaceuticals and medical devices to ensure full-process supervision and closed-loop management—enabling traceability of origin and destination, risk prevention and control, and accountability—thereby guaranteeing the quality of pharmaceuticals and medical devices.Integrate residents’ electronic health records to ensure real-time updates and seamless integration with electronic medical records. Residents can view their personal electronic health records at any time via the Qujing Tong app, interact with treating physicians, avoid duplicate tests, and reduce the burden on patients. Actively coordinate with provincial medical insurance departments to achieve interoperability with the medical insurance system through API integration, enabling patients to use third-party payment functions for medical insurance payments, and utilizing the intelligent review features of the medical insurance information system to supervise prescriptions issued by medical institutions.Promote data sharing and operational coordination among the city’s medical insurance, pharmaceutical, and healthcare sectors to form a unified “three-sector” network. 4. Promote the construction of “Smart Hospitals.”Prioritize the development of a “three-in-one” smart hospital information system integrating electronic medical records, smart services, and smart management. Deepen the development of “Internet Plus Healthcare,” treat information technology as a priority area in hospital infrastructure, and gradually improve service systems such as time-slot appointment-based medical care, internet hospital platforms, and comprehensive supervision systems for medical practices. Actively promote the development of “internet hospitals” to expand and extend the scope of medical services. (3) Enhancing the Quality and Efficiency of Public Hospitals
1. Promote improvements in hospital management. Fully implement operational autonomy in public hospitals regarding internal personnel management, appointment of mid-level cadres, recruitment and talent introduction, as well as internal performance evaluation and distribution. Continue to promote the implementation of the Chief Financial Officer (CFO) system in public hospitals at the secondary level and above, and encourage public hospitals to hire legal advisors to enhance the scientific, standardized, refined, and law-based levels of hospital operations and management. Explore the implementation of an annual salary system for Party Committee Secretaries, Hospital Directors, and CFOs, with annual salaries to be borne by the corresponding level of government.Implement the “Qujing City Implementation Plan for Promoting the Development of the Health and Medical Talent Pool” (Quban Tong [2022] No. 22) and the “Qujing City ‘Zhuyuan Talent Plan’ Special Implementation Rules for Health and Medical Talent” (Qudang Rencai [2021] No. 5). Prioritize both increasing the quantity and improving the quality of talent, strengthen the recruitment of high-level health and medical professionals, establish a system where members of hospital leadership teams provide support and services to high-level talent, and address talent shortages.Establish mechanisms for positive incentives and accountability-driven accountability. Treat key tasks such as medical quality and safety management and hospital infection prevention and control as critical indicators in hospital grading evaluations and public hospital performance assessments, and strengthen the application of assessment results. 2. Comprehensively strengthen financial management. Regularly advance the “Year of Financial Management” initiative in public medical institutions, standardize and strengthen government procurement management, and promote the deep integration of operational and financial management.Regularly disclose hospital financial information. Improve internal audit systems and strengthen internal control mechanisms covering authorization and approval, budgeting, assets, accounting, government procurement, and information disclosure. Strengthen comprehensive budget management in public hospitals, emphasizing the “Four Comprehensives and Two Reinforcements”—implementing budget management that is comprehensive in scope, process, and coverage, encompassing all human, financial, and material resources, and spanning every stage from budget formulation, approval, execution, monitoring, adjustment, final accounting, analysis, to evaluation.Timely adjust and refine budget management systems, mechanisms, processes, methods, and standards; strengthen budgetary discipline; and enhance comprehensive budget performance management. Based on changes in the external policy environment and hospital economic activities, precisely control cost expenditures, improve operational efficiency, promote the effective allocation and use of resources, and further alleviate the financial burden of medical care for the public. 3. Improve the operational management system. Establish an operational management system in all public hospitals at the secondary level and above across the city. Through scientific management, enhance the overall operational and refined management levels of hospitals. Establish a performance management and evaluation system based on the labor value and workload of medical staff, integrating efficiency, effectiveness, quality, and management, to promote high-quality development in hospital operational management. At the same time, leverage information technology to build an operational decision-making platform at the hospital level and an operational supervision platform at the government level, thereby contributing to the overall enhancement of management capabilities in the city’s health sector.(Responsible Units: Municipal Health Commission, Municipal Big Data Center, Municipal Finance Bureau; People’s Governments of all counties, cities, and districts, and the Qujing Economic and Technological Development Zone Administrative Committee, each responsible according to their respective duties)
4. Improve the performance evaluation mechanism. Comprehensively implement performance assessments for public hospitals, gradually include secondary-level comprehensive private hospitals in the scope of performance assessments, and continuously optimize the assessment indicator system. The focus will be on evaluating indicators such as medical quality, operational efficiency, sustainable development, and satisfaction ratings. Assessment results will serve as an important basis for evaluating and selecting Party branch secretaries, hospital directors, and members of the leadership team in public hospitals, fully leveraging the role of performance assessments as a “guide” and “catalyst.”The performance evaluation results of public hospitals for the previous year shall be categorized into four grades: “Excellent,” “Good,” “Qualified,” and “Unqualified.” Based on these results, the proportion of the previous year’s medical service revenue—after deducting costs and setting aside various funds and development reserves in accordance with regulations—that may be allocated for staff rewards shall be 100%, 90%, or 80%, respectively, but shall not exceed 60%.(Responsible Units: Municipal Health Commission, Municipal Party Committee Organization Department; People’s Governments of all counties, cities, and districts, and the Qujing Economic and Technological Development Zone Management Committee shall assume responsibility in accordance with their respective duties)
(IV) Unleashing New Drivers for High-Quality Development of Public Hospitals
1. Improve and refine the medical reform policy framework. Deeply study and promote the Sanming medical reform experience, institutionalize reform measures into policy regulations, and formulate and issue city-wide guiding documents on deepening medical and health system reform to provide policy support for deepening medical reform and the high-quality development of public hospitals. 2. Implement adjustments to medical service pricing and reforms in medical insurance payment methods. Conduct an assessment of medical service price adjustments at least once a year. Reasonably determine the total volume of price adjustments; within this total, prioritize key areas and implement adjustments with both increases and decreases. Accelerate the improvement of pricing policies for traditional Chinese medicine services and “Internet+medical services.” By the end of 2025, the relative pricing relationships of medical services will be further rationalized.Adhere to the principle of “determining expenditures based on revenue, achieving a balanced budget, and maintaining a slight surplus” for the basic medical insurance fund. Improve the total budget management of the medical insurance fund and strengthen the negotiation mechanism between medical insurance administrative agencies and medical institutions.Coordinate the advancement of medical insurance payment method reforms, accelerate the implementation of Diagnosis-Related Group (DRG) payment, and simultaneously refine diversified and composite medical insurance payment methods—such as payment for outpatient surgery, payment per bed-day, and fee-for-service—that accommodate the characteristics of different diseases and services. Implement policy preferences for traditional Chinese medicine and improve medical insurance payment support policies suited to its characteristics.We will continue to advance the reform of per-capita bundled payment for integrated county-level medical consortiums, strengthen supervision and evaluation, and promote the decentralization of high-quality medical and health resources and the formation of an orderly healthcare delivery system. 3. Regularly conduct centralized bulk procurement of pharmaceuticals and medical supplies. Fully implement national and provincial policies on centralized bulk procurement of pharmaceuticals and medical supplies, carry out municipal-level centralized bulk procurement, and gradually expand the scope of procurement. Implement the policy allowing medical insurance funds saved through centralized procurement to be retained, incentivizing medical institutions to use selected products reasonably and prioritize them, thereby reducing the financial burden on patients.Conduct or participate in at least one centralized volume-based procurement of pharmaceuticals and medical supplies annually. By the end of 2025, the number of generic drug names covered by centralized volume-based procurement shall exceed 450. 4. Deepen the reform of personnel and compensation systems in public hospitals. Continue to deepen the reform of staffing quota management in Grade III A-class hospitals, scientifically and reasonably determine the total staffing requirements for such hospitals, advance the reform of the filing system for staffing quotas in county-level public hospitals, and explore pilot programs for total staffing management.Implement reforms to the remuneration system for public hospitals, strictly enforce the “two allowances” policy, reasonably determine and dynamically adjust remuneration levels, and fully mobilize the initiative of medical staff. (5) Establish a New Culture for High-Quality Development in Public Hospitals
1. Enhance the patient experience. Fully implement time-slot appointment-based medical services, multidisciplinary consultations, high-quality nursing services, hospital volunteer services, and one-stop comprehensive services; explore the development of unaccompanied wards and the implementation of in-consultation (bedside) billing; and promote the mutual recognition of laboratory and diagnostic test results. Establish multidisciplinary outpatient clinics and implement systems for multidisciplinary joint consultations and rounds.Gradually expand the scope of outpatient surgery and increase the proportion of outpatient surgeries among elective procedures to improve the public’s healthcare experience. By the end of 2025, the proportion of outpatient surgeries at municipal-level Grade III Class A general hospitals shall reach 25% or higher. Promote the widespread adoption of electronic health cards for residents. 2. Build a distinctive hospital culture. 3. Care for and Support Medical Personnel. Implement the spirit of the “Guiding Opinions on Establishing a Long-Term Mechanism for the Protection, Care, and Support of Medical Personnel” (Guo Wei Ren Fa [2021] No. 13) issued by the National Health Commission, the Ministry of Human Resources and Social Security, and the Ministry of Finance. Ensure the implementation of systems for medical personnel in public hospitals regarding study, work, rest, and policy-based paid leave, and safeguard the legitimate rights and interests of medical personnel.Implement the employee convalescence system. Encourage public hospitals to support the professional development of young medical personnel through various means, such as establishing youth study funds. Improve the employee care and assistance mechanism to effectively address the practical difficulties faced by medical personnel. Deepen the development of “Safe Hospitals,” implement security accountability systems, strengthen personnel, physical, and technological security measures, improve hospital security systems, enhance smart security capabilities, and reinforce hospital safety precautions.Optimize mechanisms for the prevention and resolution of medical disputes; strictly crack down on all types of medical-related illegal and criminal activities in accordance with the law; bring medical disputes under the rule of law; and earnestly implement third-party medical liability insurance. (6) Uphold and Strengthen the Party’s Comprehensive Leadership Over Public Hospitals
1. Implement the system of hospital directors’ responsibility under the leadership of the Party Committee. Integrate Party leadership throughout the entire process, all fields, and every stage of promoting high-quality development in public hospitals. Strictly implement the system of hospital directors’ responsibility under the leadership of the Party Committee, and focus on establishing a working mechanism characterized by unified leadership by the Party Committee, division of labor and cooperation between Party and administrative bodies, and coordinated operations. Vigorously carry out the “Party Building Leads Healthcare, Zhuyuan White-Coated Pioneers” thematic campaign to enhance the quality of grassroots Party building work in public hospitals.Promote the deep integration of Party building work with medical services. Implement the “Three Cultivation” mechanism—cultivating Party members into key personnel, key personnel into Party members, and key Party members into cadres—and intensify efforts to cultivate “dual leaders.” The proportion of such leaders in secondary public hospitals has seen a significant increase. Party organizations in public hospitals shall bear primary responsibility for Party building work; the Party branch secretary of the hospital is the primary person responsible for Party building work, as well as for hospital safety and ideological work; other members of the leadership team shall implement the “dual responsibilities” system; strengthen the construction of hospital discipline inspection and supervision organizations; and comprehensively advance Party building and Party conduct and integrity in public hospitals.Implement the inspection system for public hospitals, incorporating Party building, Party conduct and integrity, hospital safety, and ideological work into the main content of inspections, and using these as important criteria for hospital grading evaluations, annual performance assessments, and cadre selection. 3. Comprehensively advance the construction of “Integrity Hospitals.”Using the slogans “Show Your Identity, Uphold Your Image, Set an Example” and “Let Local Incidents Serve as a Warning to Those Around Us” as guiding principles, comprehensively advance the construction of “Integrity Hospitals” and establish “Seven-Zero Departments” (zero tolerance for accepting red envelopes, zero tolerance for accepting kickbacks, zero tolerance for insurance fraud, zero tolerance for excessive medical treatment, zero tolerance for accepting gifts in violation of regulations,“zero tolerance” for profit-driven patient referrals, and “zero tolerance” for cold, indifferent, or pushy treatment), to foster a political environment within the hospital characterized by integrity and uprightness. We will continue to address corruption and work style issues in the healthcare sector, organizing large-scale hospital inspections and “follow-up reviews.”We will intensify efforts to foster a “culture of integrity” and thoroughly promote education on Party discipline and laws. By the end of 2025, misconduct and corruption in the medical sector will be further curbed, and a strong ethos of integrity in medical practice will be further entrenched. 4. Implement the comprehensive supervision system. Improve a diversified comprehensive supervision system that combines institutional self-governance, industry self-discipline, government oversight, and social supervision. Ensure that medical institutions fulfill their primary responsibility for self-management, establish organizational systems for lawful practice, and conduct annual self-inspections on lawful practice.Establish and improve working mechanisms for routine joint law enforcement and joint “double-random” inspections involving departments such as health and wellness, market regulation, medical insurance, and public security. Based on medical institutions’ self-inspections, routine supervision and inspections, handling of complaints and reports, and case investigations, adopt a problem-oriented approach to intensify supervision and law enforcement regarding medical services. III. Support Measures
(1) Implementing Work Responsibilities. All localities and units shall regard promoting the high-quality development of public hospitals as a key task in deepening the reform of the medical and health system. They shall strengthen organizational leadership, enhance coordination and collaboration, and ensure that all parties fulfill their responsibilities. All localities and units shall earnestly perform their duties, form a synergistic working force, and advance the high-quality development of public hospitals.
(2) Fulfill Funding Responsibilities. Implement the government’s six major funding policies for public hospitals that comply with regional health plans in accordance with regulations, and implement preferential funding and support policies for traditional Chinese medicine hospitals, infectious disease hospitals, psychiatric hospitals, children’s hospitals, and maternal and child health care hospitals.
(3) Strengthen Monitoring and Evaluation. Establish a city-wide evaluation indicator system for the high-quality development of public hospitals, and integrate it organically with performance evaluations of public hospitals. Counties (cities, districts), and economic and technological development zones shall evaluate the high-quality development of public hospitals within their jurisdictions in accordance with the principle of territorial jurisdiction, fully taking into account the actual circumstances of public hospitals at all levels and of all types, and avoiding a “one-size-fits-all” approach.
(4) Summarize and Promote Best Practices. Municipal and county-level health administration departments shall, in conjunction with relevant departments, strengthen research and guidance to identify, summarize, refine, and promote exemplary practices, thereby driving tangible results in the high-quality development of public hospitals citywide through a “point-to-area” approach.
